where to get it. This can be difficult and sometimes we find ourselves going from practitioner to practitioner
with little relief. Other times, we have experienced some recovery or restoration of our health, but are seeking more.
With a Doctorate in Naturopathy and accreditations as a Nutritionist, and Herbalist,
my desire is to teach you self health care.
To meet this goal, I am available for phone consultations during normal business hours and
off hours to help accommodate your schedule.

- Convenient manner in which to address nutritional and lifestyle behaviors that may be aggravating an acute
or chronic health issue.
- For those unable to find convenient qualified nutritional and lifestyle counseling in their area.
How phone consultation works:
- Arrange an appointment time via phone.
- Download, print out and fill out the appropriate intake forms for your first phone consultation.
- Return the forms via mail or fax (408-264-3086) prior to your phone consultation.
- On arranged appointment time and date, I will call you at your designated phone number.
During the first phone consultation you will be discussing the following:
- History of illness
- Symptoms and duration of illness
- Aggravating and alleviating factors
- Current dietary intake and lifestyle factors
- You will receive a treatment plan, referrals and recommendations as needed
during of your consultation
Deposit, cancellation and no-show policy:
- A credit card number is required for securing your first office appointment.
- You will be charged for your full visit time if you do not provide 24 hours notice of your cancellation.
- Cancellation policy applies to same-day appointments.
- 24 hours notice required for all subsequent cancellations.
Phone Consultation Fee Schedule:
- $125 for each 50 minute session.
- Session is always a minimum 50 minutes because patients are scheduled on the hour. If a session longer than 50 minutes is
required, please make that clear at the time the appointment is made.
- Payment methods accepted: check, Visa, Mastercard, American Express, and Discover (payment in in advance).
